Five factors that determine the price
1. The fistula's type and complexity
A single-tract, superficial fistula and a branched one that crosses the muscle or has been operated on before are entirely different jobs in terms of time, technique, and equipment. This is the number one reason the price range is so wide.
2. Need for imaging
While an exam may be enough for a simple fistula, we request an MRI for complex or recurrent fistulas to map the tunnel. MRI adds to the total cost, but it's always cheaper than the cost of a poorly planned surgery.
3. The chosen method
Laser (FiLaC) and other muscle-sparing techniques are priced differently from classical fistulotomy because they require special equipment and experience. On the other hand, being a same-day procedure reduces hospital stay and dressing costs — the calculation shouldn't be based on the procedure fee alone.
4. Anesthesia and hospital conditions
The standard of the facility where the procedure is performed, the type of anesthesia, and, if needed, an overnight observation stay all affect the total.
5. The surgeon's experience
I'll write this without hesitation, even about my own profession: fistula is a disease notorious for recurrence, and experience is the best insurance against a second surgery. The most expensive fistula surgery is a repeat surgery.
Public insurance (SGK) and private health insurance
Fistula treatment is a medical necessity, not a cosmetic procedure. It can be performed under SGK coverage at public hospitals. Most private health insurance plans cover fistula surgery; for methods like laser, I'd recommend asking your insurer about the "specialized equipment/technology" clause in your policy.
